EHCP Annual Review Feedback

Please complete this survey about your EHCP Annual Review Meeting which you have recently taken part in. It will take about five minutes to complete. This will be a review of your plan if you are a child or young person.

This will be your child’s plan if you are the parent/carer. This will be the review of a child or young person with an EHCP who you support if you are a professional. How does this make an impact?

Your answers will help us know what is working well and what could be changed to make things better. We let families know twice a year what the survey results say from everyone who has responded. We will make sure nobody can tell which family said what. The anonymous results will be published on the Local Offer website
